Hari AIDS Sedunia ini pertama kali diusulkan oleh James W. Bunn dan Thomas Netter di WHO pada tahun 1987, dengan tujuan untuk memaksimalkan liputan media agar meningkatkan kesadaran masyarakat tentang HIV AIDS.

Hari AIDS Sedunia diperingati pertama kali pada 1 Desember 1988.

Kemudian pada tahun 1991, pita merah diperkenalkan menjadi simbol universal untuk meningkatkan kesadaran tentang HIV/AIDS, simbol tersebut diperkenalkan oleh sekelompok seniman di New York.
